Mental Health Skill Building
Focus is on psycho-social education and symptoms of mental illness, verbal processing and expression groups such as managing one’s own emotions, learning new coping skills, anger and stress management, the importance of a daily medication regime to enjoy increased quality living and support to maintain recovery.
The Licensed Clinical Social Worker or Licensed Professional Counselor and the team will determine what the individual’s service needs are in terms of their diagnosis.
Individual counseling will be initiated by the LCSW or LPC and the QMHP. The individual will discuss with QMHPs their perception of current situations and possible solutions.
Coordination of services with other contracted providers and physicians will be initiated and documented in the individual’s service plan. Consumer under psychiatric care will continue to receive these services.
A Licensed Clinical Social Worker will be contracted to provide and monitor mental health services.

Life Skills
Support and training focuses on the individual’s need to develop new methods of learning to cope with various stressors of everyday living, journal writing, exercising control over their emotions, building self-esteem to promote self-confidence and self-determination, as well as learning to advocate for themselves.
Support to achieve accomplishments in this area will include:
- Providing opportunities for individuals to discuss alternate means of expressing themselves other than through anger
- Training in functional skills of personal care and activities of daily living (toileting, bathing, grooming, dressing, and eating, mobility, communication, household chores, food preparation, money management, shopping, etc.);
- Training in functional skills related to use of community resources (transportation, shopping, restaurants, social and recreational activities, etc.);
- Building cohesiveness and bonding within a group
- Helping consumer to build a support base
- Initiating daily interaction among peers and staff to increase appropriate socialization skills
- Promoting increased self-esteem to help consumer feel better about themselves
Providing opportunities for role modeling
Emergency and Stabilization Services
A critical incident report will be filed in the case of an emergency or critical incident. A copy of the critical incident report will be placed in the consumer’s case record and forwarded to the referring source.
A preliminary treatment plan will be developed within 72 hours following admissions to identify the consumer’s immediate needs for goals of support and training.
Housing
Synergy strives to battle homelessness. We partner with area landlords that offer apartments, shared homes, and single family homes. Homes are based on availability and resources.
Here at Synergy Health Systems we offer transitional housing to our consumers that are homeless or have the need for housing. To be accepted into housing you have to be referred to us through case management or by an agency. We provide housing in a safe environment so that you can jump start your life. Our rooms include bedroom furniture and all utilities paid. We have many homes in the Tidewater area.

Nurses
At Synergy Health Systems registered nurses coordinate all aspects of care: health care services, home medical equipment, and infusion therapy. The hours of nursing services may include short-term/intermittent; extended care; private duty; and around-the-clock. On home visits, our nurses practice patientfocused family-centered care that encompasses a patient’s physical and emotional health. In addition, nurses with specialty experience are available including:
- Medical Clinic
- HIV/AIDS
- Pediatrics
- Perinatal, including maternal/infant
- Rehabilitation
- Geriatrics
- Infusion
Occupational Therapists
These professionals help patients restore functioning, develop strength and coordination, retrain nerve pathways, and learn compensation techniques. Occupational Therapists also provide observations and assessments of the patient’s condition and instruction to the patient/family on self management Techniques.
Physical Therapists
These professionals provide skilled therapy interventions in the treatment of the patient’s illness, rehabilitative needs and preventive care. They use a holistic approach in providing ongoing assessments, problem identification, specific therapeutic interventions and teaching and training activities.
Speech/Language Pathologists
Speech language pathologists evaluate patients and provide speech/language services in the home.
Scope of activities includes assessment and treatment of disorders of the voice, language, hearing, swallowing; administration of diagnostic tests; audio logic screening; instruction and training in use of alternative communications systems; development of a written maintenance plan; supervision and instruction of patient and family.
Home Health Aides
Home Health Aides at Synergy deliver basic personal care, check vital signs such as pulse and temperature, and monitor the patient’s nutrition and fluid intake. Home health aides also assist the patient with activities of daily living from bathing and personal care to light housekeeping and meal preparation. They also will spend time with patients, through such activities as talking together, reading or listening to music.
